Electronic Notice to Airport Users Form (E-Form)​: O'Hare​​ ​| Midway​

Facilities Site Investigation Request (Smartsheet)

Before and During Construction, the General Contractors, Consultants, or Design Teams must fill out the Site Investigation Request form on Smartsheets. See Instructions(opens in new tab)​.

Facilities Project Coordination Request (Smartsheet)​

All Mechanical, Electrical, Plumbing, Controls, and Fire Protection coordination requests must be initiated with a minimum of 72 hours advance notice via this form on Smartsheets. See Instructions(opens in new tab).​​

This form is for tenants requesting documentation from CDA for use in their design and submit it to CDA Point of Contact for required approvals.

Company Vehicle Access Form

This is the Badging and Compliance form for companies requesting vehicle access to airfields.

Certificate of Liability Insurance(opens in new tab)

​All city contractors and subcontractors must provide a copy of the COI indicating “The City of Chicago and all of its designated representatives" as additional insured. Insured amounts should match requirements dictated in the Concessionaire's / Tenant's lease / license documents. Insurance required of Subcontractors: Tenant and/or Contractor must name Subcontractor(s) as a named insured(s) under Tenant and/or Contractor's insurance or Tenant and/or Contractor will require each Subcontractor(s) to provide and maintain Commercial General Liability, Commercial Automobile Liability, Worker's Compensation and Employers Liability Insurance and when applicable Excess/Umbrella Liability Insurance. Insured coverage amounts should match outlined requirements dictated per Concessionaire / Tenant or Sub-Tenant lease / license documents.​
